Adoption Request Form

This form is to be used for Adoptions Requests ONLY!

Send me the name of a missing American Serviceman whose cause I can champion!

We are not offering an exclusive adoption of anyone, to anyone.  It is important that you realize we need as many people as possible involved in the homecoming of each still-missing hero.

If you are not an adult, please make sure giving this information and adopting is OK with your parent(s)/guardian(s).

Determining preferences:
Choosing between a military branch and home state, please indicate the criteria that is of the most importance to you. We will try to accommodate that criteria if there is a POW/MIA who meets it. In selecting an adoptee, your primary consideration is:
If you select branch of service, you must select a branch.
If you select home state, you must enter your state in your address, above.

Selecting between branches of service, your preferred branch is:
It is strongly recommended that you adopt a missing hero from the state where you currently reside because your elected representatives may take a greater interest in a POW/MIA from their own state (where you would be voting).
Your type of adoption:
Examples of Civilian/Corporate would be a local business, a school class, a fire department, etc. A Military/Veterans Organization would be a local chapter of an organization like: VVA, VFW, ROTC, Army Reserve Unit, etc.

If this is a group or organization adoption, you must include your group/org name, local chapter, etc. in comments below or this will be treated as a Personal/Family adoption. (Approx. size of membership would be appreciated !)

Adoption of Category 5 personnel:

This category deals with the degree of expected enemy knowledge which states that category 5 is unrelated to degree of enemy knowledge. This often means the loss did not occur during combat, such as a loss at sea due to accident.

It further states that it represents individuals whose remains have been determined to be non recoverable as outlined in Department of the Army Technical Manual 10-286, January 1964, section 39. So nobody is looking for them. If the category was assigned in error (or even if they were correctly listed as category 5) no one is working their case, yet category 5's have been found before!

Please let us know if you are willing to adopt someone with a Category 5 rating.

We do not usually recommend that your first adoption be a category 5. If that is not the case, can we send you someone matching your other criteria who is listed as a Category 5?

NOTE: You will receive a confirmation email containing the information you provide on this form. Please!!! Be sure to give your full, valid e-mail address! An email address does NOT include www. or http://  It follows a format of: something@somewhere.end.  The end will usually be .com or .net or .org or .mil or .gov or .edu or .info or .tv but could be something else.  An email address will NEVER contain any spaces. (Most folks know this but we DO get a significant percentage of requests from people just starting on the Internet.)

If you don't get the this email from us, you won't get the adoption you requested, either.

This form is to be used for Adoptions ONLY!
